Good Life Groups for Parents: Helping Your Child Step Into Adult Health Care (Session 2 of 2)

Learn more and register HERE

Adult health care doesn’t come with instructions – especially for children and youth with disabilities or special health care needs.

Whether your child has complex medical needs or just needs help managing a basic check-up, all young people need support learning to navigate adult health care. But too often, families are left without guidance, facing big changes with little preparation.

You don’t have to figure it out alone.
Join a supportive virtual group of Minnesota parents and caregivers to explore practical, family-centered tools for health care transition using the Charting the LifeCourse framework. The group is facilitated by certified CtLC Ambassadors who are also parents of children with disabilities and have earned Good Life Group facilitator badges.

In this Good Life Group, you’ll:
– Map out what your child needs for a healthy future
– Use tools to support growing independence or ongoing care
– Practice communicating with doctors on behalf of or with your child
– Explore how to stay involved as roles and systems change
– Share real-life strategies with other parents
– Take steps toward a smoother, more supported transition
