Charting the LifeCourse™ is a set of tools designed to help families and individuals think about what they need now and in the future. Use these resources to set goals, identify supports, and create a vision for a good life.

Family Voices of Minnesota – Charting the LifeCourse

Family Voices of Minnesota staff are all parents of children with disabilities and serve as CtLC ambassadors and Good Life group facilitators. We offer one-to-one guidance to help families use CtLC tools to plan for the future, get unstuck, and focus on their child and family’s well-being. We also host parent groups to practice using the tools and connect with other caregivers.

Good Life Group Events

These interactive peer learning collaboratives offer hands-on support for parents and family members of children with disabilities. Participants learn to use the Charting the LifeCourse (CtLC) framework and tools to imagine and work toward a “good life” for their child and family.

Exploring the Life Stages – Charting the LifeCourse™

The "Exploring the Life Stages" section of the Charting the LifeCourse (CtLC) framework offers tools and guides to help families plan for each life stage—from prenatal through aging. These resources include Quick Guides and the "Experiences and Questions" booklet, designed to assist families in envisioning a good life and identifying supports needed at every stage.

A resource for families who want to support their child’s independence while still being involved in important decisions. It offers tools to help parents and youth explore options, communicate choices, and plan together — including sample agreements and worksheets like the Decision-Making Supports Tool and Integrated Supports Star.

Caregiver Well-Being (LifeCourse Tools)

Resources for parents and caregivers to support their own health, balance, and quality of life while caring for a child or family member with disabilities. Includes tools to help identify strengths, plan for the future, and connect with supports.

Disability Hub MN – Charting the LifeCourse™

Disability Hub MN provides Charting the LifeCourse (CtLC) tools for families and lists CtLC-related events on their website. Families and providers can access worksheets, guides, and training opportunities to support planning and decision-making.
